What type of information we have
We currently collect and process the
following information:
- Personal
identifiers, contacts and characteristics (for example, name, date of birth and
contact details) - A brief history
of presenting concerns - A summary of
therapy sessions
How we get the information and why we do we have
it
Most of the personal information we
process is provided to us directly by you or your carer (if you are under the
age of 18) for one of the following reasons:
- An enquiry for
therapeutic support - To provide
details for someone you care for to receive therapeutic support - As a
professional, for someone you work with to access support
We also receive personal information
indirectly, from the following sources in the following scenarios:
- Someone working
with you or looking after you who feels you may benefit from therapeutic
support
Under the General Data Protection
Regulation (GDPR), the lawful bases we rely on for processing this information
are:
(a) Your consent (or
consent of a parent/care whereby the subject is under the age of 18). You are
able to remove your consent at any time. You can do this by contacting
rebecca@lilypadplaytherapy.co.uk
(b) We have a
contractual obligation – we are working with you to provide therapeutic support.
(c) We have a legitimate interest.
Initial information is collated on
enquiry or referral. This information is
retained to ensure the support requested is able to be offered.
Clients who access therapeutic support
through Lily Pad Play Therapy Ltd. will have some information recorded and
retained. This information includes
their name, date of birth, presenting concerns, contact details (including
those of parents/carers) ‘scores’ from a questionnaire and a brief history of
concerns. A brief summary of each
session is recorded. This information
will include the date of the session, length of the session, limited
information about what items were used and any themes identified by the
therapist. This information is retained
to support the therapist in facilitating the therapy. This information is not shared.
Anonymised information in relation to
the presenting concern of the client and session activity may be used for
research purposes. This is with consent
only and this data will be anonymous.
What we do with the information
We use the information that you have
given us in order to;
- Support
therapeutic interventions - To respond to
enquiries
We may share some information with
Play Therapy UK (PTUK)
and the Association of Play and Child Psychotherapists (APAC) for the purpose of research. This information will be anonymise and shared
only with consent for the purposes of research.
How we store your information
Contact enquiries through our website
will be stored on our password protected encrypted system. This information will only be kept for as
long as is needed to support your enquiry.
Referral information will be stored on
a secure database. Should therapy not
take place, this information will be deleted following notice that the therapy
is no longer required or is not taking place.
Clients attending therapy will have
their information stored on a secure system called Fortuna. This is a system devised by Play Therapy UK specifically
for Play Therapists and complies with General Data Protection Regulation (GDPR)
and meets the requirements of the Data Protection Act 2018.
Information will be retained for 10
years after which time the client record will be deleted.
Your data protection rights
Under data protection law, you have
rights including:
Your right of access – You have the right to ask us for copies of
your personal information.
Your right to rectification – You have the right to ask us to rectify
information you think is inaccurate. You also have the right to ask us to
complete information you think is incomplete.
Your right to erasure – You have the right to ask us to erase your
personal information in certain circumstances.
Your right to restriction of processing – You have the right to ask us to restrict
the processing of your information in certain circumstances.
Your right to object to processing – You have the the right to object to
the processing of your personal data in certain circumstances.
Your right to data portability – You have the right to ask that we transfer
the information you gave us to another organisation, or to you, in certain
circumstances.
You are not required to pay any charge
for exercising your rights. If you make a request, we have one month to respond
to you.
